PARENT: by entering name below, I agree to the rules above for the vendor listed and will provide adult supervision for said vendor at all times. Further it is understood I, the undersigned, hereby agrees to waive any liability against and indemnify Mundelein Park & Recreation District, Mundelein Community Connection, its officers, directors, employees, and managing agent from and against all claims, damages, losses, judgments, executions and expenses arising out of or resulting from my participation in organized events. I hereby acknowledge that my participation in any events is at my sole risk and I shall not hold anyone liable for any injuries or loss I sustain as a result of my participation or spectatorship in said activities. Further, this waiver and indemnification includes, but is not limited to, claims (a) attributable to bodily injury, sickness, disease or death, or to injury to or destruction of tangible property, including the loss of use resulting therefrom and (b) caused in whole, or in part, by any negligent act or omission.
